Job Overview
We are seeking a compassionate and dedicated Visiting Nurse to join our healthcare team. In this role, you will provide high-quality patient care in the comfort of patients' homes. As a Visiting Nurse, you will assess patients' health needs, develop care plans, and collaborate with other healthcare professionals to ensure optimal patient outcomes. This position requires strong clinical skills and a commitment to delivering personalized care.
Responsibilities
- Conduct comprehensive health assessments for patients in their homes.
- Develop, implement, and evaluate individualized care plans based on patient needs.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' progress and adjust care plans as necessary.
- Educate patients and their families about health management and disease prevention.
- Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
- Collaborate with interdisciplinary teams to coordinate patient care effectively.
- Provide emotional support and counseling to patients and their families.
Skills
- Proficiency in medical terminology to communicate effectively with healthcare professionals.
- Strong patient care skills, demonstrating empathy and compassion in all interactions.
- Valid nursing license with relevant experience in home health or community nursing preferred.
- Excellent communication skills, both verbal and written, for effective patient education.
- Ability to work independently while managing time efficiently in a home setting.
- Strong critical thinking skills to assess situations and make informed decisions regarding patient care.
